Patient Information

Basic patient information such as identifier, life status (alive/deceased), date of birth, sex, indication for referral, and name (in PhenoTips Enterprise instances), are recorded in the “Patient information” section. If you wish to obtain measurement charting and interpretation in the Measurements section of the patient’s form, it is crucial to enter the date of birth and the patient’s sex .

The “Indication for referral” section can be used to record a chart summary or referral notes. All text in this section will be analyzed to suggest standardized phenotypes using the Human Phenotype Ontology (HPO) in the Clinical Symptoms and Physical Findings section. DO NOT record any protected health information in this section.
